LEGISLATIVE RESOLUTION honoring Celeste Carnes upon the occasion of her
designation as recipient of the Provider of the Year Award by the Rural
Health Network of Oswego County
WHEREAS, It is the custom of this Legislative Body to recognize and pay
tribute to those individuals within the health field who distinguish
themselves through professional excellence and who have made significant
contributions to the quality of life of citizens in the State of New
York; and
WHEREAS, Attendant to such concern, and in full accord with its long-
standing traditions, this Legislative Body is justly proud to honor
Celeste Carnes upon the occasion of her designation as recipient of the
Provider of the Year Award by the Rural Health Network of Oswego County;
and
WHEREAS, This auspicious award recognizes outstanding individuals in
the health field for their service to the rural health needs of their
community and who embody the spirit and ideals and values of the Rural
Health Network; and
WHEREAS, A certified family nurse practitioner with Northern Oswego
County Health Services, Inc., Celeste Carnes is a shining example of a
person dedicated to serving her patients and her community; and
WHEREAS, In 1996, Celeste Carnes earned her certification as a family
nurse practitioner from Community General Hospital in Syracuse, and her
associate degree in nursing from Onondaga Community College; she is a
member of the Nurse Practitioner's Association of New York State and is
also certified with the American Academy of Nurse Practitioners Program;
and
WHEREAS, Celeste Carnes has served in school based health centers
since its inception in 1996, first with Sandy Creek School in Sandy
Creek, New York, and in 2001, moving to the Pulaski Academy and Center
School Based Health Clinic where she continues to provide care today;
and
WHEREAS, As a leader in her profession, Celeste Carnes has proven
herself to be a great asset to the school based health clinic program
and in addressing needs of rural health in Oswego County, demonstrating
her love for the students and her patients at the health clinic; and
WHEREAS, It is the sense of this Legislative Body that when individ-
uals of such noble aims and accomplishments are brought to our atten-
tion, they should be celebrated and recognized by all the citizens of
this great Empire State; now, therefore, be it
RESOLVED, That this Legislative Body pause in its deliberations to
honor Celeste Carnes upon the occasion of her designation as recipient
of the Provider of the Year Award by the Rural Health Network of Oswego
County; and be it further
RESOLVED, That a copy of this Resolution, suitably engrossed, be tran-
smitted to Celeste Carnes.
