LEGISLATIVE RESOLUTION honoring Dr. Hal Teitelbaum upon the occasion of
his designation as recipient of the 2016 Ottaway Medal by the Orange
County Citizens Foundation

WHEREAS, It is the sense of this Legislative Body to recognize that the
quality and character of life in the communities across New York State
are reflective of the concerned and dedicated efforts of those organiza-
tions and individuals who would devote themselves to the welfare of the
community and its citizenry; and
WHEREAS, This Legislative Body is justly proud to honor Dr. Hal
Teitelbaum upon the occasion of his designation as recipient of the 2016
Ottaway Medal by the Orange County Citizens Foundation to be celebrated
at its Annual Dinner on Wednesday, May 4, 2016; and
WHEREAS, The Orange County Citizens Foundation is a non-profit organ-
ization dedicated to improving the lives of Orange County residents by
bringing people together to preserve the county's environmental and
cultural heritage; and
WHEREAS, The Ottaway Medal is given each year to an individual who has
displayed extraordinary achievement and outstanding service to the
community; and
WHEREAS, A visionary in the medical field, Hal Teitelbaum completed
his training in Internal Medicine at New York Hospital-Cornell Medical
Center and both Medical Oncology and Hematology at Memorial Sloan-Ket-
tering Cancer Center; he is also Board Certified in these three special-
ties; and
WHEREAS, In 1998, Dr. Hal Teitelbaum earned his Master of Business
Administration degree from Columbia Business School, and received his
Juris Doctor from Columbia Law School four years later; and
WHEREAS, Today, Dr. Hal Teitelbaum, is the Founder, Managing Partner
and CEO of Crystal Run Healthcare, a premier multi-specialty group and
one of New York State's most progressive medical practices, with more
than 300 physicians and over 400 providers in total practicing 40
specialties across the region; and
WHEREAS, This remarkable health care leader has strived to bring new
economic development to the Hudson Valley Region while also building a
new model for accessible quality health care which improves patient
outcomes while reducing costs; and
WHEREAS, During his illustrious tenure, Dr. Hal Teitelbaum has guided
Crystal Health with the belief that physicians have a professional
imperative to act as stewards of patient welfare, and that they have a
responsibility to help establish and contribute to the development of a
high value, sustainable health care system; and
WHEREAS, In 2015, Dr. Hal Teitelbaum oversaw the launch of Crystal Run
Health Plans, a health insurance company which provides quality, afford-
able health insurance and HMO products to the communities it serves; and
WHEREAS, In addition, Dr. Hal Teitelbaum helped establish Crystal Run
Transformation Services, a company which provides management services to
guide health care organizations through the complex processes required
in today's current environment; and
WHEREAS, An integral part of numerous civic and professional organiza-
tions, Dr. Hal Teitelbaum is a member of the Medical Group Management
Association, the New York State Bar and the CEO Council of the American
Medical Group Association; he is also a member with Advanced Standing in
the American Association for Physician Leadership; and
WHEREAS, Throughout the entire period of his community service, a
period of constructive involvement, Dr. Hal Teitelbaum has stood
constant in dignity, good grace and humor; and

WHEREAS, With him throughout has been his wife, Jennifer, who feels
privileged to be a part of his life and rejoices in his achievements;
and
WHEREAS, Rare indeed is the impressive dedication shown by an individ-
ual for the benefit of others which Dr. Hal Teitelbaum has displayed
throughout his life; now, therefore, be it
RESOLVED, That this Legislative Body pause in its deliberations to
honor Dr. Hal Teitelbaum upon the occasion of his designation as recipi-
ent of the 2016 Ottaway Medal by the Orange County Citizens Foundation;
and be it further
RESOLVED, That a copy of this Resolution, suitably engrossed, be tran-
smitted to Dr. Hal Teitelbaum.
