        CONGRATULATING  Anne Constantino upon the occasion
        of her retirement after 37  years  of  distinguished
        service to Horizon Corporations
 
  WHEREAS,  It  is the sense of this Legislative Body to recognize and
honor those distinguished citizens of this great Empire State  who  have
devoted  themselves  to  faithfully  serving the best interests of their
community and the needs of its residents; and
 
  WHEREAS, Attendant to such concern, and  in  full  accord  with  its
long-standing  traditions,  this  Legislative  Body  is  justly proud to
congratulate Anne Constantino upon the occasion of her retirement  after
37  years  of  distinguished  service  to Horizon Corporations; she will
officially retire from the esteemed position of  President  and  CEO  in
June of 2023; and
 
  WHEREAS,  Anne Constantino began her illustrious career with Horizon
Corporations in 1986 as a senior counselor at the  organization's  first
addiction clinic; and
 
  WHEREAS,  As  the company grew, Anne Constantino moved into clinical
and operations  leadership  roles  and  was  appointed  to  her  current
position as President and Chief Executive Officer in 1994; in this role,
she  leads  the  development  and implementation of strategic and annual
operating plans for Horizon Health Services, Horizon Village and Horizon
Health Alliance; and
 
  WHEREAS, This extraordinary woman is credited with  guiding  Horizon
through  expansion  to  serve  more counties and creating innovative and
sustainable support services for patients; the organization now  has  20
locations  in Western New York, a tele-network across the state, and two
residential and inpatient campuses in Erie and Niagara counties; and
 
  WHEREAS, Furthermore, Horizon staff  has  grown  to  more  than  800
clinical and administrative members through this development period; and
 
  WHEREAS,  Anne  Constantino  has  been an integral part of Horizon's
leadership team since 2006 while serving as its Chief Financial Officer;
prior to serving this vital organization, Anne spent 16 years in Ernst &
Young's health care consulting division where she  conducted  turnaround
projects; and
 
  WHEREAS,  Under  the able leadership of Anne Constantino, and with a
rigorous focus on a workplace culture of respect and inclusion,  Horizon
has  been  selected  as  a New York State Best Companies to Work for the
past 14 years; and
 
  WHEREAS, In addition to her role  with  Horizon  Corporations,  Anne
Constantino  is  the  founding  member  and  current  President of Value
Network, a membership organization of Western New  York  behavioral  and
health  care  providers  working  on  transformation  of the health care
system to improve patient outcomes and decrease costs; and
 
  WHEREAS, Anne Constantino  holds  a  Master  of  Science  degree  in
Counseling  and  Educational  Psychology,  and  received  her Leadership
Coaching  Certification  from  the  University  of  Buffalo  School   of
Management, Center for Leadership and Organizational Effectiveness; and
 
  WHEREAS,   With   a  steadfast  and  unwavering  commitment  to  her
community, Anne Constantino is an  active  member  of  several  industry
organizations  and  Boards  including:  NYS Governor's Opioid Settlement
Funds  Board,  March  2022-present;  Executive  Committee  member,   NYS
Addiction   Services   Providers,   2011-present;  Executive  Committee,
Coalition for Community Services, A Network of New  York  SUD  treatment
agencies,  2010-present;  and  Amherst  Chamber  of  Commerce, Executive
Committee; and
 
  WHEREAS,  For  nearly  four  decades,  Anne   Constantino   rendered
faithful,  conscientious  and  valuable service to Horizon Corporations,
truly earning the admiration, esteem and affection  of  her  colleagues;
and
 
  WHEREAS,  Rare  indeed  is  the  occurrence  of such a compassionate
blending of strength, leadership,  intellect,  and  commitment  as  that
demonstrated  by  Anne  Constantino  over  a  lifetime  of sacrifice and
dedication to others; now, therefore, be it
 
  RESOLVED, That this Legislative Body pause in its  deliberations  to
congratulate  Anne  Constantino upon the occasion of her retirement, and
to wish her well in all her future endeavors; and be it further
 
  RESOLVED, That a copy of this  Resolution,  suitably  engrossed,  be
transmitted to Anne Constantino.
