MAYER -- read once and referred to the Committee on Aging AN ACT to amend the elder law, in relation to requiring the director of the office for the aging to develop materials on social adult day care programs and provide such information to hospital discharge coordina- tors THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM- BLY, DO ENACT AS FOLLOWS: Section 1. Subdivision 2 of section 215 of the elder law is amended by adding a new paragraph (d) to read as follows: (D) THE DIRECTOR SHALL DEVELOP AND DISTRIBUTE MATERIALS TO HOSPITAL DISCHARGE COORDINATORS WITH INFORMATION REGARDING SOCIAL ADULT DAY CARE PROGRAMS. SUCH MATERIALS SHALL INCLUDE, BUT NOT BE LIMITED TO, ADMISSION CRITERIA AND THE BENEFITS AND LOCATIONS OF SUCH PROGRAMS. S 2. This act shall take effect immediately. MAYER -- read once and referred to the Committee on Aging -- committee discharged, bill amended, ordered reprinted as amended and recommitted to said committee AN ACT to amend the elder law, in relation to requiring the director of the office for the aging to develop materials and making available a list of programs and services offered by local area agencies on aging; and to amend the public health law, in relation to requiring hospitals to provide information relating to aging services to certain patients upon discharge THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM- BLY, DO ENACT AS FOLLOWS: Section 1. Section 203 of the elder law is amended by adding a new subdivision 11 to read as follows: 11. (A) THE DIRECTOR SHALL DEVELOP A LIST OF PROGRAMS AND SERVICES OFFERED BY LOCAL AREA AGENCIES ON AGING ALONG WITH CONTACT INFORMATION FOR THE LOCAL AREA AGENCIES ON AGING AND NYCONNECTS ORGANIZED BY COUNTY. (B) THE INFORMATION DEVELOPED PURSUANT TO PARAGRAPH (A) OF THIS SUBDI- VISION SHALL BE MADE AVAILABLE ON THE OFFICE'S WEBSITE AND PROVIDED TO HOSPITAL DISCHARGE COORDINATORS LOCATED IN THE STATE. S 2. Title 1 of article 29-D of the public health law is amended by adding a new section 2997-f to read as follows: S 2997-F. PROVISION OF INFORMATION RELATING TO AGING SERVICES. WHEN- EVER A HOSPITAL IS DISCHARGING AN INDIVIDUAL AGED SIXTY YEARS OF AGE OR OLDER, THE HOSPITAL'S DISCHARGE COORDINATOR SHALL PROVIDE SUCH INDIVID- UAL WITH A COPY OF THE INFORMATION PROVIDED TO THEM PURSUANT TO SUBDIVI- SION ELEVEN OF SECTION TWO HUNDRED THREE OF THE ELDER LAW THAT CORRE- SPONDS TO THE INDIVIDUAL'S COUNTY OF RESIDENCE.
