Session II will offer information about the 4M framework for a patient-centered health system designed to meet the needs of older adults. The 4 Ms of good health care are: 1) what Matters to you, 2) Mobility, 3) Mentation, cognitive and mental health wellbeing, and 4) Medications to support your health goals. Presenters will share the work of the John A.
