AN ACT to amend the insurance law and the public health law, in relation
  to the use of insurance identification cards

  THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND  ASSEM-
BLY, DO ENACT AS FOLLOWS:

  Section  1. Section 3217-a of the insurance law is amended by adding a
new subsection (f) to read as follows:
  (F) EACH INSURER SUBJECT TO THIS ARTICLE SHALL ISSUE TO  EACH  INSURED
AN  INSURANCE  IDENTIFICATION  CARD  WHICH  SHALL  INCLUDE THE FOLLOWING
INFORMATION:
  (1) THE INSURED'S IDENTIFICATION INFORMATION, INCLUDING THE  INSURED'S
NAME AND IDENTIFICATION NUMBER;
  (2)  THE NAME OF THE PRODUCT THE INSURER USES TO DESCRIBE THE BENEFITS
COVERED UNDER THE CONTRACT;
  (3) THE AMOUNT OF THE INSURED'S CO-PAYMENT, IF ANY; AND
  (4) THE BILLING ADDRESS AND CONTACT INFORMATION, INCLUDING A TELEPHONE
NUMBER, FOR THE INSURER.
  EACH INSURER SUBJECT TO THIS ARTICLE SHALL ISSUE A NEW  CARD  TO  EACH
INSURED  WHENEVER ANY OF THE INFORMATION REQUIRED TO BE INCLUDED ON SUCH
CARD CHANGES.
  S 2. Section 4324 of the insurance law is  amended  by  adding  a  new
subsection (f) to read as follows:
  (F) EACH HEALTH SERVICE, HOSPITAL SERVICE OR MEDICAL EXPENSE INDEMNITY
CORPORATION  SUBJECT  TO  THIS ARTICLE SHALL ISSUE TO EACH SUBSCRIBER AN
INSURANCE IDENTIFICATION CARD WHICH SHALL INCLUDE THE FOLLOWING INFORMA-
TION:
