Makes provisions requiring every application, petition or request submitted for a variance, amendment change of zoning approval of a plat or change of a land use regulation to have appended thereto a disclosure statement setting forth any interest of a state officer, municipal officer, or municipal employee in such application.

AN ACT to amend the general municipal law, in relation to disclosure  in
  certain  applications  of  conflicts of interest of municipal officers
  and employees

  THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND  ASSEM-
BLY, DO ENACT AS FOLLOWS:

  Section  1. Subdivision 1 of section 809 of the general municipal law,
as added by chapter 646 of the laws of  1969,  is  amended  to  read  as
follows:
  1.  [Every]  EACH  MUNICIPALITY  SHALL  REQUIRE THE INCLUSION IN EVERY
application, petition, or request submitted for a  variance,  amendment,
change  of zoning, approval of a plat, exemption from a plat or official
map, license, or permit[,] pursuant to the provisions of any  ordinance,
local  law,  rule,  or  regulation  constituting the zoning and planning
regulations of a  municipality  A  STATEMENT,  WHICH  shall  [state]  BE
APPENDED  THERETO,  INDICATING  the  name, residence, and the nature and
extent of the interest of any state officer or any officer  or  employee
of  such municipality or of a municipality of which such municipality is
a part, in the person, LIMITED LIABILITY COMPANY, partnership, or  asso-
ciation  making  such  application,  petition,  or  request (hereinafter
called the applicant), to the extent known to such  applicant.  FOR  THE
PURPOSE  OF THIS SECTION, A MUNICIPALITY SHALL BE DEEMED TO BE A PART OF
THE COUNTY WITHIN WHICH IT IS LOCATED.
  S 2. Section 809 of the general municipal law is amended by  adding  a
new subdivision 2-a to read as follows:
  2-A.  THE STATEMENT DISCLOSING AN INTEREST IN THE APPLICATION SHALL BE
PROVIDED TO EACH APPLICANT AND SHALL CONTAIN THE INFORMATION  AND  SHALL
BE IN THE FORM SET FORTH IN THIS SUBDIVISION:
