AN ACT to amend the public health law, in relation to  providing  copies
  of redacted death certificates

  THE  PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM-
BLY, DO ENACT AS FOLLOWS:

  Section 1. Paragraph (g) of subdivision  1  of  section  4174  of  the
public  health  law,  as  amended by chapter 396 of the laws of 1989, is
amended and a new paragraph (h) is added to read as follows:
  (g) upon request of a  board  of  elections,  issue  certification  of
death[.]; AND
  (H)  UPON  REQUEST, AND AT A FEE TO BE DETERMINED BY THE COMMISSIONER,
FURNISH COPIES OF REDACTED DEATH CERTIFICATES WITH THE FOLLOWING  INFOR-
MATION OMITTED: THE NAME, NUMBER AND STREET OF RESIDENCE, SOCIAL SECURI-
TY NUMBER, MEDICAL RECORDS NUMBER AND DATE OF BIRTH OF THE DECEDENT, AND
THE  NAMES  OF  THE  DECEDENT'S  MOTHER,  FATHER  AND  SURVIVING SPOUSE,
PROVIDED, HOWEVER, THAT THE REDACTED CERTIFICATE SHALL  REFLECT  WHETHER
OR NOT THE DECEDENT HAD A SPOUSE.
  S 2. This act shall take effect immediately.
