AN ACT to amend the public health law, in relation  to  the  content  of
  birth certificates

  THE  PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM-
BLY, DO ENACT AS FOLLOWS:

  Section 1. This act shall be known and may be  cited  as  "the  modern
family birth certificate act".
  S  2. Section 4132 of the public health law is amended by adding a new
subdivision 5 to read as follows:
  5. EACH PARENT OF THE CHILD MAY CHOOSE TO BE DESIGNATED ON THE CERTIF-
ICATE OF BIRTH AS "FATHER" OR "MOTHER", AS APPLICABLE, OR AS "PARENT".
  S 3. This act shall take effect on  the  first  day  of  January  next
succeeding  the  date  upon  which  it shall have become a law; provided
that, effective immediately,  the  commissioner  of  the  department  of
health shall promulgate appropriate rules and regulations for the imple-
mentation of the provisions of this act on or before its effective date.
