TITLE OF BILL:

An act to amend the public health law, in relation to establishing the
Take A Look medical and dental workforce employment tour pilot program;
and providing for the repeal of such provisions upon expiration thereof

 
PURPOSE OR GENERAL IDEA OF BILL:

The purpose of this bill is to.boost the pool of health care workers,
including physicians, dentists, physician assistants, nurse practition-
ers, nurse anesthetists, and nurse midwives practicing in underserved
areas in Upstate New York.

 
SUMMARY OF PROVISIONS:

Section 1: Adds section 2958-b of the public health law to establish the
Take A Look medical and dental workforce employment pilot program for
three years to be implemented by an association representing upstate
medical and/or dental facilities to conduct a campaign to promote and

market the health care workforce employment and career opportunities
available in the central, north country, western, finger lakes, capital
region, mohawk and southern tier economic development regions of the
state. Funds appropriated shall be available for a multi-media campaign
and also utilized to promote and educate individuals on the availability
of funding under various loan forgiveness programs and may be utilized
to conduct "Take A Look" tours within the identified economic develop-
ment regions. The implementing association will provide an annual report
on or before the first day of November offering an analysis of the shor-
tage of health care workers in the identified regions, and such report
shall be made public on the websites of the departments of health and
economic development. § 2: establishes the effective date and repeal
date.

 
JUSTIFICATION:

This bill re-establishes and expands the "Take A Look" workforce employ-
ment program conducted in previous years by the Iroquois Healthcare
Association. The proposed expansion to the pilot program will now
include marketing to dentists because, even though New York dental
schools graduate approximately 750 dentists annually - among the highest
numbers of any state - New York continues to struggle with unmet need
for Medicaid recipients and other vulnerable populations. Several barri-
ers contribute to the challenges of providing access to dental care to
low-income, rural or developmentally disabled individuals, including too
few dentists practicing in rural and high need areas,' a net annual
out-migration of New York dentists,'- a decline in CODA*-accredited
dental residency programs,3 the exorbitant costs of attending dental
school4 and establishing a dental practice, perennially insufficient
Medicaid reimbursement rates, and a wave of dentist retirements in the
wake of the pandemic.

Many out-of-staters, Downstaters, and even Upstate New York locals, do
not know about the professional and lifestyle opportunities available to
them in Upstate New York. That lack of awareness remains a significant
barrier to attracting a sufficient health care workforce.  In fact, New
York State currently has 156 dental Health Practitioner Shortage Areas
(HPSAs), including 92 designated HPSA facilities and 64 designated HPSA
population , groups (mostly Medicaid eligible populations).5 This means
that each cf these designations has a population-to-provider ratio of at
least 4000 people to 1 dentist.  Because NYS currently meets only 16.68%
of the documented need, the federal Health Resources and Services Admin-
istration (HRSA) estimates that New York needs an additional 1,100
dentists to remove all of its dental HPSA designations.6

The Take A Look pilot program will specifically address the need to
educate healthcare professionals on the opportunities available to them
and the, desirability of living in the Upstate regions, including the
central, north country, western, finger lakes, capital region, mohawk
and southern tier economic development regions.

 AN ACT to amend the public health law, in relation to  establishing  the
   Take  A  Look  medical  and  dental  workforce  employment  tour pilot
   program; and providing for the repeal of such provisions upon  expira-
   tion thereof

   THE  PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM-
 BLY, DO ENACT AS FOLLOWS:
 
   Section 1. Article 29-A of the public health law is amended by  adding
 a new title 1-B to read as follows:
                                 TITLE 1-B
   TAKE A LOOK MEDICAL AND DENTAL WORKFORCE EMPLOYMENT TOUR PILOT PROGRAM
 SECTION 2958-B. TAKE A LOOK MEDICAL AND DENTAL WORKFORCE EMPLOYMENT TOUR
                   PILOT PROGRAM.
   §  2958-B.  TAKE  A  LOOK MEDICAL AND DENTAL WORKFORCE EMPLOYMENT TOUR
 PILOT PROGRAM. 1. THE COMMISSIONER  SHALL,  WITHIN  MONIES  APPROPRIATED
 THEREFOR, ESTABLISH THE TAKE A LOOK MEDICAL AND DENTAL WORKFORCE EMPLOY-
 MENT  PILOT  PROGRAM  TO  BE  IMPLEMENTED BY AN ASSOCIATION REPRESENTING
 UPSTATE MEDICAL AND/OR  DENTAL  FACILITIES  TO  CONDUCT  A  CAMPAIGN  TO
 PROMOTE  AND  MARKET  THE  HEALTH  CARE  WORKFORCE EMPLOYMENT AND CAREER
 OPPORTUNITIES IN THE FOLLOWING ECONOMIC  DEVELOPMENT  REGIONS:  CENTRAL,
 NORTH  COUNTRY, WESTERN, FINGER LAKES, CAPITAL REGION, MOHAWK AND SOUTH-
 ERN TIER WHERE THERE IS A CRITICAL  SHORTAGE  OF  HEALTH  CARE  WORKERS,
 INCLUDING  PHYSICIANS AND DENTISTS, WHICH IMPEDES ACCESS TO VITAL HEALTH
 CARE SERVICES AND IMPAIRS THE ECONOMIC  STATUS  OF  THE  COMMUNITIES  IN
 THESE REGIONS.
   2.  A.  FUNDS  SHALL  BE MADE AVAILABLE FOR A MULTIMEDIA CAMPAIGN THAT
 WILL ENCOURAGE RESIDENTS OF THE REGIONS IDENTIFIED IN SUBDIVISION ONE OF
 THIS SECTION TO TRAIN FOR AND ENTER THE HEALTH WORKFORCE IN SUCH IDENTI-
 FIED REGIONS, TO ATTRACT INDIVIDUALS UNDERGOING TRAINING IN HEALTH  CARE
 PROFESSIONAL  EDUCATIONAL  PROGRAMS INCLUDING GRADUATE MEDICAL EDUCATION
 TRAINING AND GRADUATE DENTAL EDUCATION TRAINING IN OTHER REGIONS OF  NEW
 YORK STATE AND OTHER STATES TO PURSUE CAREER OPPORTUNITIES IN SUCH IDEN-
 TIFIED  REGIONS,  AND TO ATTRACT AND ENCOURAGE HEALTH CARE PROFESSIONALS

 FROM OTHER STATES TO PURSUE  CAREER  OPPORTUNITIES  IN  SUCH  IDENTIFIED
 REGIONS.
   B. FUNDING SHALL BE UTILIZED TO PROMOTE AND EDUCATE INDIVIDUALS ON THE
 AVAILABILITY OF FUNDING UNDER THE NEW YORK STATE DOCTORS ACROSS NEW YORK
 PROGRAM,  THE  PRIMARY  CARE SERVICE CORPS, THE REGENTS LOAN FORGIVENESS
 PROGRAM, AND THE NATIONAL HEALTH SERVICE CORPS.
   C. FUNDING SHALL BE UTILIZED TO CONDUCT "TAKE  A  LOOK"  TOURS  WITHIN
 SUCH IDENTIFIED ECONOMIC DEVELOPMENT REGIONS. "TAKE A LOOK" TOURS AIM TO
 EXPOSE  UNDERGRADUATE  MEDICAL AND DENTAL STUDENTS, PHYSICIAN AND DENTAL
 RESIDENTS, PHYSICIAN ASSISTANTS,  NURSE  PRACTITIONERS,  NURSE  ANESTHE-
 TISTS,  AND NURSE MIDWIVES IN TRAINING ACROSS THE STATE AND FROM OUTSIDE
 THE STATE TO PRACTICE OPPORTUNITIES IN  UPSTATE  NEW  YORK  COMMUNITIES.
 EACH  TOUR  ITINERARY  PROVIDES  OPPORTUNITIES  FOR RESIDENTS TO "TAKE A
 LOOK" AND LEARN ABOUT UPSTATE NEW YORK  HOSPITALS,  MEDICAL  AND  DENTAL
 STAFFS,  COMMUNITIES  AND  CULTURES THROUGH HOSPITAL, CLINIC, PHYSICIAN,
 AND DENTAL OFFICE VISITS.
   3. A. THE IMPLEMENTING  ASSOCIATION  SHALL  PROVIDE  ANNUALLY,  ON  OR
 BEFORE  THE FIRST DAY OF NOVEMBER, AN ANALYSIS OF THE SHORTAGE OF HEALTH
 CARE WORKERS IN THE ECONOMIC DEVELOPMENT REGIONS IDENTIFIED IN  SUBDIVI-
 SION  ONE  OF THIS SECTION, AN ACCOUNTING OF THE MULTIMEDIA CAMPAIGN AND
 THE IMPACT OF THE "TAKE A LOOK" TOURS ON SUCH SPECIFIED ECONOMIC  DEVEL-
 OPMENT  REGIONS. SUCH REPORT SHALL INCLUDE AT LEAST THE FOLLOWING INFOR-
 MATION:
   (I) THE NUMBER OF INDIVIDUALS WHO PARTICIPATED IN "TAKE A LOOK"  TOURS
 OVER THE PREVIOUS TWELVE MONTHS;
   (II)  THE NUMBER OF "TAKE A LOOK" TOUR PARTICIPANTS WHO BEGAN PRACTIC-
 ING IN ONE OF THE ECONOMIC DEVELOPMENT REGIONS IDENTIFIED IN SUBDIVISION
 ONE OF THIS SECTION WITHIN TWELVE MONTHS  FOLLOWING  TOUR  PARTICIPATION
 AND THE LOCATION OF SUCH PRACTICE;
   (III)  THE  NUMBER  OF  PATIENTS  SERVED  IN EACH ECONOMIC DEVELOPMENT
 REGION BY SUCH PARTICIPANTS WITHIN THE TWELVE MONTHS FOLLOWING THE START
 OF THEIR PRACTICE;
   (IV) A DESCRIPTION OF THE IMPACT OF THE  PROGRAM  ON  THE  COMMUNITIES
 WHERE SUCH PARTICIPANTS BEGAN PRACTICING; AND
   (V)  SUCH OTHER INFORMATION AS THE COMMISSIONER MAY DEEM NECESSARY AND
 APPROPRIATE.
   B. SUCH REPORT SHALL BE PROVIDED TO THE CHAIRS OF THE  CENTRAL,  NORTH
 COUNTRY, WESTERN, FINGER LAKES, CAPITAL REGION, MOHAWK AND SOUTHERN TIER
 REGIONAL  ECONOMIC  DEVELOPMENT COUNCILS, THE TEMPORARY PRESIDENT OF THE
 SENATE, THE SPEAKER OF THE ASSEMBLY AND THE COMMISSIONER  AND  SHALL  BE
 MADE  PUBLICLY  AVAILABLE ON THE DEPARTMENT'S WEBSITE AND THE WEBSITE OF
 THE DEPARTMENT OF ECONOMIC DEVELOPMENT.
   4. TO THE EXTENT OF FUNDS APPROPRIATED  THEREFOR,  MEDICAL  ASSISTANCE
 FUNDS,  INCLUDING  ANY FUNDING OR SHARED SAVINGS AS MAY BECOME AVAILABLE
 THROUGH FEDERAL WAIVERS OR OTHERWISE UNDER TITLES EIGHTEEN AND  NINETEEN
 OF  THE  FEDERAL  SOCIAL  SECURITY  ACT, MAY BE USED FOR EXPENDITURES IN
 SUPPORT OF THE TAKE A LOOK MEDICAL AND DENTAL WORKFORCE EMPLOYMENT PILOT
 PROGRAM.
   § 2. This act shall take effect immediately and shall  expire  and  be
 deemed repealed three years after such date.
