 AN ACT to amend the election law, in relation to requiring signatures on
   a voter registration form to be notarized and  providing  a  fail-safe
   registration method
 
   THE  PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM-
 BLY, DO ENACT AS FOLLOWS:

   Section 1. Subparagraphs (viii) and (xii) of paragraph (k) of subdivi-
 sion 5 of section 5-210 of the election law, as amended by  chapter  113
 of  the  laws of 2023, are amended and a new subparagraph (xiv) is added
 to read as follows:
   (viii) A statement informing the applicant that if the form is submit-
 ted by mail [and the applicant  is  registering  for  the  first  time,]
 certain  information  or  documents [must] MAY NEED TO be submitted with
 the mail-in registration form [in order to avoid additional  identifica-
 tion  requirements upon voting for the first time]. Such information and
 documents [are] MAY INCLUDE:
   (A) [a driver's license or department  of  motor  vehicles  non-driver
 photo ID number; or
   (B)  the  last four digits of the individual's social security number;
 or
   (C)] a copy of a current and valid GOVERNMENT ISSUED photo identifica-
 tion; [or
   (D)] AND
   (B) a copy of a  current  utility  bill,  bank  statement,  government
 check,  paycheck  or  other  government document that shows the name and
 address of the voter.
   (xii) A place for the applicant to execute the form on a line which is
 clearly labeled "signature  of  applicant"  preceded  by  the  following
 specific form of affirmation:
   AFFIDAVIT: I swear or affirm that:
      * I am a citizen of the United States.
      * I will have lived in the county, city, or village for at least 30
        days before the election.

                       [ ] is old law to be omitted.
