HEAP & Heartshare Applications

The HEAP application is now available to download. The Heartshare Application will be available on February 6, 2023. Interested applicants should call the office at 718-984-4073 upon receipt of their HEAP certification to apply for Heartshare.
March 30th will be the last day that HEAP applications can be submitted through my office. The HEAP representative will be here (3845 Richmond Ave) on March 30th from 10am to 3pm.
