The HEAP application is now available to download.  The Heartshare Application will be available on February 6, 2023. Interested applicants should call the office at 718-984-4073 upon receipt of their HEAP certification to apply for Heartshare.

March 30^th will be the last day that HEAP applications can be submitted through my office. The HEAP representative will be here (3845 Richmond Ave) on March 30th from 10am to 3pm.
