To nominate a veteran for recognition, please complete the form below no later than Thursday, September 10th. Because this information will be used to create a commemorative booklet, we ask that, whenever possible, the section labeled "Nominee Information" be completed by the nominated veteran.
We also accept posthumous nominations. In those cases, we kindly ask that the nominator complete the form to the best of their ability using the information available to them.

Dear Friend,
Throughout our communities, veterans have answered the call to serve our nation in times of war and peace. Their experiences span generations, military branches and places of service, but each has helped protect the freedoms and way of life we cherish.
Our annual Veterans Recognition Ceremony provides an opportunity to express our gratitude, share these important personal histories and ensure that the contributions of local veterans are never forgotten. As we commemorate the 250th anniversary of our nation, it is especially meaningful to recognize the men and women whose service and sacrifice have defended the freedoms we cherish. Every veteran’s story deserves to be shared, and every veteran’s service deserves our respect and appreciation.
Anyone may nominate a veteran who resides within the 54th Senate District, which includes Ontario, Livingston and Wayne counties, as well as the Monroe County communities of Chili, Churchville, Honeoye Falls, Mendon, Riga, Rush, Scottsville and Wheatland.
Over the years, nominations have been submitted by family members, friends, neighbors, fellow veterans, community members, and local organizations. I deeply appreciate everyone who takes the time to nominate a veteran for recognition. Your thoughtfulness allows us to learn more about their military service, preserve their story and honor their contributions in the presence of family, friends and community.
My respect for our veterans is deeply personal. My father served in the United States Air Force during the Korean War, and my son-in-law served in the United States Army as a Green Beret. As a member of a military family, I understand that military service touches not only those who wear the uniform, but also the loved ones who stand beside them.
I hope you will help us recognize a veteran from the 54th Senate District. It would be a privilege to welcome them, along with their family and friends, to my Veterans Recognition Ceremony on October 23 at the Fort Hill Performing Arts Center in Canandaigua.
