To nominate a veteran for recognition, please complete this form. Some sections are intended to be completed by the veteran being nominated. Because this information will be used to create a commemorative booklet, we ask that, whenever possible, those sections be completed by the nominee.
We also accept posthumous nominations. In those cases, we kindly ask that the nominator complete the form to the best of their ability using the information available to them.

Dear Friend,
Our veterans didn’t wait to be asked before they stepped up for us. Now it’s our turn to step up for them. Through my Veterans Recognition Program, I make it my mission every year to put the spotlight on the men and women whose service too often goes unheralded. Since taking office in 2017, I have proudly recognized hundreds of veterans across the 54th Senate District, and this year I want to recognize even more.
I need your help to do it. You know a veteran whose story deserves to be told, whose sacrifice deserves to be celebrated. Don’t let that recognition wait another year. Nominate them today and stand with me as we honor them in a special ceremony on October 23rd at Fort Hill Performing Arts Center in Canandaigua.
Anyone can submit a nomination, but the veteran being honored must live within the 54th Senate District: Ontario, Livingston, and Wayne Counties, along with the Monroe County communities of Chili, Churchville, Honeoye Falls, Mendon, Riga, Rush, Scottsville, and Wheatland.
I carry this mission personally. My father served in the U.S. Air Force. My son-in-law, a West Point graduate, served as a U.S. Army Green Beret. I have seen firsthand what it costs to serve, and I refuse to let that sacrifice go unnoticed. Please join me and nominate a veteran.
