BILL NUMBER:S5892 TITLE OF BILL: An act to amend the elder law, in relation to requiring the director of the office for the aging to develop materials and making available a list of programs and services offered by local area agencies on aging; and to amend the public health law, in relation to requiring hospitals to provide information relating to aging services to certain patients upon discharge PURPOSE: This legislation amends elder law to require the NYS Office for the Aging to develop and provide information to hospitals regarding aging agency programs. SUMMARY OF PROVISIONS: Section 1: Amends section 203 of the elder law to add a new subdivision 11 which requires the Director of the State office for the Aging to develop materials on local area aging agency and NYConnects programs and make these materials available on the NYSOFA website and to provide such materials to hospital discharge coordinators. VALESKY -- read twice and ordered printed, and when printed to be committed to the Committee on Rules AN ACT to amend the elder law, in relation to requiring the director of the office for the aging to develop materials and making available a list of programs and services offered by local area agencies on aging; and to amend the public health law, in relation to requiring hospitals to provide information relating to aging services to certain patients upon discharge THE PEOPLE OF THE STATE OF NEW YORK, REPRESENTED IN SENATE AND ASSEM- BLY, DO ENACT AS FOLLOWS: Section 1. Section 203 of the elder law is amended by adding a new subdivision 11 to read as follows: 11. (A) THE DIRECTOR SHALL DEVELOP A LIST OF PROGRAMS AND SERVICES OFFERED BY LOCAL AREA AGENCIES ON AGING ALONG WITH CONTACT INFORMATION FOR THE LOCAL AREA AGENCIES ON AGING AND NYCONNECTS ORGANIZED BY COUNTY. (B) THE INFORMATION DEVELOPED PURSUANT TO PARAGRAPH (A) OF THIS SUBDI- VISION SHALL BE MADE AVAILABLE ON THE OFFICE'S WEBSITE AND PROVIDED TO HOSPITAL DISCHARGE COORDINATORS LOCATED IN THE STATE. S 2. Title 1 of article 29-D of the public health law is amended by adding a new section 2997-f to read as follows: S 2997-F. PROVISION OF INFORMATION RELATING TO AGING SERVICES. WHEN- EVER A HOSPITAL IS DISCHARGING AN INDIVIDUAL AGED SIXTY YEARS OF AGE OR OLDER, THE HOSPITAL'S DISCHARGE COORDINATOR SHALL PROVIDE SUCH INDIVID- UAL WITH A COPY OF THE INFORMATION PROVIDED TO THEM PURSUANT TO SUBDIVI- SION ELEVEN OF SECTION TWO HUNDRED THREE OF THE ELDER LAW THAT CORRE- SPONDS TO THE INDIVIDUAL'S COUNTY OF RESIDENCE.
